New Patient Forms

Please click to download, carefully read, and sign the appropriate forms below prior to your first appointment. Bring the completed forms with you if we are meeting in person, or send by fax, US mail, or email if we are meeting via video/phone. If you are unable to print them, please read them and I will provide copies for you to sign.

  • All new patients must sign the Informed Consent and Privacy Practices forms.

  • Patients who wish to meet by video or phone must read and sign the Telepsychology Services form.

  • For couples therapy, both parties must read and sign the Couples Therapy Informed Consent Addendum
